SCAN offers FREE "Take Control of Your Health" Workshops

Take Control of Your Health” Workshops throughout Monmouth County

SCAN (Social Community Activities Network) in Eatontown is offering FREE workshops for adults with chronic health conditions. Take Control of Your Health (TCOYH) is a 6 week evidence-based workshop created by Stanford University. It provides you with support and essential tools necessary to help you design your own strategy for managing chronic conditions such as arthritis, heart and lung conditions, anxiety, diabetes or high blood pressure. In addition to peer support, you’ll learn practical ways to deal with pain and fatigue and discover better nutrition and exercise options. You’ll also learn more effective ways to communicate with your family and doctors about your health. TCOYH workshops are facilitated by trained Peer Leaders who also have chronic conditions. Each week participants set their own goals to improve their health and their life. All participants receive a free reference book called Living a Healthy Life with Chronic Conditions, which covers each of the weekly topics in more depth. Some topics include:

  • managing stress
  • improving your communication
  • coping with depression and fatigue
  • increasing your energy levels
  • eating healthy and maintain your weight
